Support Coordinators Australia supported coordination is here to support you and your family or carers on your NDIS journey. Our disability support coordination services are person-centred and individually tailored. We will work closely with you and your family or support people to make sure we understand you and your support needs.

What is support coordination?

Support coordination is a capacity building NDIS support which works with you to:

  • Understand and use your NDIS plan to pursue your goals
  • Connect you with the best NDIS providers, community, and other government services
  • Build your confidence and skills to use and coordinate your supports.

What are the levels of support coordination in a NDIS plan?

There are three levels of support coordination that can be included in your plan:

  • Support connection – builds your ability to connect with informal, community and funded supports so that you can get the most out of your plan and pursue your goals.
  • Coordination of supports – builds the skills you need to understand and use your plan. A support coordinator works with you to make sure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be part of your community.
  • Specialist support coordination – a higher level of support coordination for people with more complex needs and situations who need specialist support. A specialist Support Coordinator will assist you to manage changes and help ensure consistency.

What is Supported Independent Living?

Supported Independent Living (SIL) is a NDIS support package that lets you live with other people with disability, or on your own. It can include support for everyday tasks like cooking, cleaning and personal care, to build your independence.

Our SIL houses can also connect you to new activities, social events and hobbies in your home and your local community.

Support Coordinators Australia Supported Independent Living (SIL) houses

Support Coordinators Australia supports people to live as independently as possible. We will make sure your SIL house meets your support needs. We can provide support staff day and night, seven days a week if you need it.

We have Support Coordinators Australia vehicles so we can take you to appointments, the shops, community activities and social events.
